The City of Dublin requires that owners of massage establishments register all their employees with Dublin Police Services and promptly report any employee changes. The establishment owner must submit this form to Dublin Police Services to report any new additions or removals among their employees, as compared to the list of employees provided in the Massage Establishment Permit Application. Additionally, owners should utilize this form when submitting a CAMTC renewal/recertification application for massage therapists. |

To apply you must make a Livescan appointment ($39 fee) and pay the $126.00 processing fee to the City of Dublin. You must also submit the completed application with a check for the Department of Justice in the amount of $300 to complete the process. |
